Chronic Care Management

Hello Medicare Beneficiary:

 

You should have received a letter, email, text message or automated call from your physician office explaining the benefits of Medicare's Chronic Care Management program.

 

The goal of the program is to make sure you get the best possible support from everyone who is involved with your care.

 

Our care managers will work with you via phone or other methods to:

 

• Develop and manage a Comprehensive Care Plan to help you understand how to care for your conditions so that you can be as healthy as possible

• Discuss and manage your symptoms and concerns

• Manage your medications

• Assist with appointment setting and Annual Wellness Visit

• Coordinate your visits with other doctors, facilities, lab, radiology, or other testing

 

Our goal is to provide you with the best care possible to keep you out of the hospital and minimize costs and inconvenience to you due to unnecessary visits to doctors, emergency rooms, labs, or hospitals.

 

This benefit is offered by Medicare and follows their payment schedule. Your portion is typically a 20% coinsurance, which is approximately $8.00. If you have secondary insurance coverage, it may cover the coinsurance.

 

You may discontinue this service at any time for any reason. We know your time and your health is valuable and we hope that you will consider participation in the program with our practice.
